the Man had after a suicide attempt admitted to the intensive care unit of a hospital in the Valleys.
”[A loved one] in front of the staff that this is the first time [the man] will check for attempted suicide and that he needs to get a psychiatric contact/assessment before discharge. She also asks if they should keep him and see a risk that he will try again,” it says in a notification which has been made to the IVO.
But according to the notification, that the man was discharged by the doctor who then contacted the man's relatives. He had been given a time at a psychiatric reception later the same week.
he was found dead the day after having been written out. A relative writes that the man should have been allowed to remain in the hospital and directly should have received the help of psychiatry, but to wait until later in the week.
”Want to complain that you don't make a more accurate assessment that there is risk for further attempts and instead choose to write him out. It was the first time, perhaps, he had been able to get the help to feel better and thereby prevented that he ended his life,” it says in the notification.
”We're not looking for any scapegoat, without want it the next time similar happens, in the hospital, it is better practice and the skills to handle the event. Perhaps it would then have not had to stop with that he took his life.”
the Hospital is investigating the incident.
